My wholly dependent mother is permanently living at Lucknow, UP whereas I am posted at Meerut and residing with my family at Meerut. In this case, whether two plastic cards could be issued, one for my mother and other for me & my family or I have to take temporary attachment for my mother and plastic card for me & my family.
Secondly, whether, I have to include my mother name also in plastic card if two plastic cards are being issued or one plastic card and one temporary attachment is issued.
Thirdly, If I am transferred to non CGHS covered area in UP and left my family and mother at my home town Lucknow, whereat CGHS facility is available then in such situation, my family or mother can enjoy the facility of CGHS on payment of six monthly CGHS contribution, the facility presently available to employees, posted in North East/some other specific areas.

Apropos extension of validity of CGHS card in other cities, may i know whether a retired Govt servant holding CGHS card for a particular Wellness in the Delhi is eligible to avail medical facility and get medicine from any of the Wellness Centre in Delhi being all records available on line.
